CSXSRISR: Log ans Notes Returns Blank For Some Service Requests (Doc ID 2098753.1)

Symptoms

On : 12.1.3 version, Service Request Form

ACTUAL BEHAVIOR
---------------
 'LOG AND NOTES' is blank For some Service Requests even they they are available in database


EXPECTED BEHAVIOR
-----------------------
Log and Notes should show the Notes for the corresponding Service Request

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Open Create Sr form
2. Query for any existing Service Request
3. In workbench tab see that there are existing notes
4.Click Log and Notes Button
It is found to be blank
