This note provides a brief overview of - per_disabilities_f is missing two columns assignment_id and assessment_due_date

Symptoms

Problem Statement:

After applying 12.1 HRMS RUP8, when trying to access Disability information via
Employee self service the following error occurs:

Exception Details.
oracle.apps.fnd.framework.OAException: oracle.jbo.SQLStmtException:
JBO-27122: SQL error during statement preparation. Statement: SELECT * FROM
