Notifications Not Sent When Employees Submit External Learning

Neither FYI nor Approval Notifications are getting generated "sometimes" when "employees" (random different employees) submit external learning training

This is intermittent and not readily reproducible.

Note that it looks like all stopped at the same location in the workflow. At BLOCK right after completed activity "V5.0 Complete Notified activity in Approval Flow".

Retries do not work.
Current work-around is to cancel the stopped workflow which removes it from the pending list and the user re-enter the training.
When the user re-enters the training, the notifications are sent as expected.
