Symptoms
When attempting to open a notification to approve a Purchase Order the following error occurs.
ERROR
ORA-20002: 3240: Error '-6502 - ORA-06502: PL/SQL: numeric or value error: invalid LOB locator specified:

STEPS
The issue can be reproduced with the following steps:
1. Send Purchase Order for approval
2. Approver opens notification
3. Error occurs
