Symptoms
On : 12.1.3 version
When attempting to assign responsibility to a user,
the following error occurs.

(ROUTINE=FND_USER_RESP_GROUPS_API.Insert_Assignment) has been detected in FND_USER_RESP_GROUPS_API.INSERT_ASSIGNMENT
