ORA-06510: PL/SQL unhanded user-defined exception ORA-06512: at "APPS.WF_NOTIFICATION" When Approving Quality Results (Doc ID 2135191.1)

On : 12.1.3 version, Product Development Electronic Signatures

When attempting to Approve and Sign E-record for GMD ERES Results entry
the following error occurs.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. OPM Quality > Samples > Results
Scenario: Checking the behavior of entering result with the E-Signature and AME Approval.
Result Entry E-Signature will be launched since the setup for AME rule is Self Approver whoever added or updated the results will be the Approver.
Created Sample  upon Saving Disposition will be Pending
In clicking Results and Evaluation by default Evaluation is null
Upon Entering the Results if it Matches the Target value column then evaluation column will changed to Accept
in case there is no value in target upon providing result value the evaluation column will update also to accept
Approved and Signed
Click Sign button receive error above.
