Incorrect Incident Addres In Service Request When Created from iSupport

On : 12.2.4 version, Service Request

On Creating Service Request,the SR gets created with incorrect address

Expect the SR to get created with correct address

The issue can be reproduced at will with the following steps:
(R)iSupport System Administrator
1.Create Service Request
Incident Address
See something like null, and some special characters
2. Goto Next and then submit SR gets saved and Request Number gets generated
