Incorrect Incident Addres In Service Request When Created from iSupport (Doc ID 2139697.1)

On : 12.2.4 version, Service Request

ACTUAL BEHAVIOR
---------------
After creating a service request from iSupport, the service request gets created with incorrect address

EXPECTED BEHAVIOR
-----------------------
Expect the service request to be created with correct address

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
(R)iSupport System Administrator
(N)Dashboard
1. Create Service Request
2. Go to Incident Address
3. Go to Next and then submit service request
4. Service request is saved and request number gets generated
5. Query the newly created service request and notice the address is incorrect
