Field Service Gantt / Contact Center Create Customer Window: Issues with Focus and Displays after Upgrading RDBMS to 11G

After upgrading RDBMS to 11G, some issues may appear in Forms.
Regenerating the forms may not solve the issue.
1. In the Contact Center (CSCCCCRC.fmb):
(R) Customer Support
(N) Customer Management > Contact Center
- Customer Type = Person
- Click on 'Create Customer' button.
When opening the Create Customer window, there is only one field "Name", instead of the expected fields: Title, First Name, Middle and Last Name.
This particular issue will cause the following error when the user tries to save the record:
Both the first name and last name cannot be null for person record.
Please enter the first name, last name, or both.
2. In the Field Service > Dispatch Center (CSFDCMAI.fmb):
a) When refreshing the Gantt, no data is returned back.
b) Customer Name is no populated.
