Web ADI Upload Error: A problem occurred opening the file for reading: java.io.IOException: File not saved to server. (Doc ID 2145802.1)

Symptoms

Seeded and custom layouts are getting the following Web ADI upload errors:

1.

WebADI upload process terminate with below error.

A problem has been encountered initializing the control for importing data as detailed below. The data import has been aborted.
A problem occurred opening the file for reading: java.io.IOException: File not saved to server.
Number of valid rows
BNE_ROWCOUNT

bne_log_file.txt shows the error:

BneCreateDocHolderBean.doSetIncoming() error processing bne:map: oracle.apps.bne.exception.BneFatalException: Unable to find valid Mapping.
