Symptoms

 The following message is seen in the concurrent program log for a concurrent request: 

Beginning post-processing of request xxxxx on node yyyy at DD-MON-2YYYY 00:00:00
Post-processing of request xxxxx failed at DD-MON-2YYYY 00:00:00 with the error message:
One or more post-processing actions failed. Consult the OPP service log for details.
