Price List Qualifier Results in Error: FRM-92101: There Was a Failure in the Forms Server During Startup (Doc ID 2148133.1)

Symptoms


After R12 Upgrade and Qualifier value From value set issue

Price List :XXXXC USD TRANSFER , Qualifier : Shipping Org , Value : XXXC Org
Price List : XXXXD USD TRANSFER PO , Qualifier : Procurement Org , Value : XXXD Org

While access qualifier Value From value set from price list form and qualifier group form, application is getting disconnected with form error.
 
This is happening for seeded qualifier attributes, especially for qualifier Contexts of Global Procurement, Intercompany : Selling/Shipping Operating Unit, Procurement/Receiving Operating Unit.

Trying to add a qualifier to a price list receive error:

ERROR

FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration. Please look into the web-server log file for details.



STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Oracle Pricing Manager
2. Price Lists/Price List Setup
3. Qualifiers Qualifier context : Global Procurement , Attribute : Procuring Operating Unit
4. Get the error
FRM-92101: There was a failure in the Forms Server
during startup. This could happen due to invalid
configuration.
Please look into the web-server log file for details.
