Flexible Spending Account (FSA) and Healthcare Savings Account (HSA) Deductions not Taken

Employees that do not touch benefits during Open Enrollment are not having FSA/HSA deductions taken.

FSA Elements should be generated appropriately per pay period the same amount that was being taken in prior year

The issue can be reproduced at will with the following steps:
1. Run Open life event.
2. FSA's default to "Enrolled" option.
3. Review Rates form from Benefits Service Center.
  > See Estimated Rates show as expected
  > Element Entry shows no per pay period amount
