E-Business Suite 12.2 Applications DBA, ADOP Utilities issues

When running autoconfig for E-Business Suite 12.2 after online patching it completes with errors:

"txkGenADOPWrapper.pl

SQL> SELECT ad_zd_adop.get_node_type('test1') FROM DUAL
*
ERROR at line 1:
ORA-01422: exact fetch returns more than requested number of rows
ORA-06512: at "APPS.AD_ZD_ADOP", line 2958"
