Symptoms
1) Import Standard Purchase Orders show error
The request finished with an error and produced the following completion message:
Concurrent program returned no reason for failure
2) The request log does not show it completing

3) The concurrent request table does contain a process id
select oracle_process_id
from fnd_concurrent_requests
where request_id = &Request_number
This returns a NULL for oracle_process_id
