Concurrent Processing R12 : Concurrent Managers Do Not Start And Throws Error "Could Not Initialize The Service Manager FNDSM" (Doc ID 2160835.1)

On EBS 12.1.3 version, Concurrent Managers are not starting and Internal Manager log shows the following error :-

Cause: insert_fcp failed due to ORA-01403: no data found
ORA-06512: at "APPS.FND_CP_FNDSM", line 134
ORA-06512: at line 1.

The SQL statement being executed at the time of the error was: and was executed
Could not initialize the Service Manager FNDSM_XXXX. Verify that XXXXX has been registered for concurrent processing.
Routine AFPEIM encountered an error while starting concurrent manager STANDARD with library /xxxx/xx_home/oracle/apps/apps_st/appl/fnd/12.0.0/bin/FNDLIBR.


 Upgrade Solaris OS from 10 to 11 and Database from 11g to 12c versions.
