Information Entered During Activity Creation Not Showing On Activity Association Screen (Doc ID 2169093.1)

Symptoms

ACTUAL BEHAVIOR

When creating an activity, users enter Activity Cause, Activity Source, Activity Type and Shutdown Type. The notification comes up saying the activity is created.
However, the fields are blank in both Activity Association form and Activity Workbench.


EXPECTED BEHAVIOR

All fields (Activity Cause, Activity Source, Activity Type and Shutdown Type) should be populated in both Activity Association form and Activity Workbench.


STEPS

The issue can be reproduced at will with the following steps:
1. Responsibility: Maintenance Super User.
2. Navigate to Activities > Activity Workbench.
3. Click New button to create a new activity.
4. Complete the Activity screen, filling out Activity Source, Activity Cause, Activity Type and Shutdown Type.
5. Click Save. Receive notification that the Activity has been created.
6. Navigate to Activities > Activity Association. Find the Activity that was just created.
7. Associate an Asset. Click on Work Order Defaults. Activity Source, Activity Cause, Activity Type and Shutdown Type are blank. Need to fill in again.
8. Navigate to Activities > Activity Workbench. Enter in Activity and click Launch. Go to Results (Activities) and select Activity. Do the Asset Association here.
    Enter in the Asset Number and Activity. Activity Source, Activity Cause, Activity Type and Shutdown Type are blank. Need to fill in again.
