Requisitions Not Approving With Workflow Error ORA-06502: PL/SQL: numeric or value error: character string buffer too small (Doc ID 2170639.1)

Symptoms

On 12.2x version for Requisitions only getting error while approving and the  Requisition remains in Incomplete status

NOTE:  This only occurs when no approver is found in the approval list.  If approvers are found successfully the error does NOT occur. 
 
STEPS
The issue can be reproduced at will with the following steps:
1. Upgrade from 12.1.3 to 12.2.5
2. Create and submit Requisition for approval
3.  Error occurs when trying to generate No Approver Found notification
