PL/SQL Errors Executing File oklbpvip.sql. During Application of Pre-Install Patches for 12.2 Upgrade

When attempting to upgrade to 12.2 the following error occurs.

Merged patch failed with oklbpvip.sql.

When trying to apply patch 10124646 with 19796566 and preinstall patches based on the note 1448102.2, all the workers failed with the following error:
sqlplus -s APPS/***** @/r03/acct/abag/fs1/EBSapps/appl/ad/12.0.0/patch/115/sql/adsqlwrapper.sql '/r03/acct/abag/fs1/EBSapps/appl/okl/12.0.0/patch/115/sql/oklbpvip.sql &un_okl &batchsize 14 18'

PL/SQL procedure successfully completed.

ORA-06550: line 75, column 9:

PL/SQL: ORA-00947: not enough values
ORA-06550: line 79, column 11:
PL/SQL: SQL Statement ignored

The issue can be reproduced at will with the following steps:
1. Upgrade to 12.2 from 12.1.3.
