Flexfields Segments for Category Structure Code Are Not Showing Up In Form with error FRM-40735 ORA-01403

Symptoms
ACTUAL BEHAVIOR
---------------
Flexfields Segments are not showing up in Form
User setup a Category and tried to migrate category codes. However, they were not able to view those segments created earlier.
When they open the flexfields it throws an error "FRM-40735:POST-QUERY trigger raised unhandled exception ORA-01403".
Further, while trying to create them freshly, it throws message as 'Duplicate Record'
EXPECTED BEHAVIOR

Expect to be able to view/update categories
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) See below error in Key Flexfields KFF for Inventory : Item Categories
FRM-40735: POST-QUERY trigger raised unhandled exception ORA-01403
2) In PIM Item Catalog :
PIM Responsibility ---> Setup Workbench----> Catalogs----> --->Categories
There is no data for created category
