R12 Patch Wizard Impact Analysis Report Does Not Show Impacted Files Due To Error 'WARNING: Patch metadata LDT file not loaded' And 'Truncation of attribute VERSION' Before The Details Button For Certain Patches Is Not Displayed

Symptoms
E-Business Suite 12.1.3 Applications Manager, Patch Application Issues
Patch wizard impact analysis report is not showing results for some patches. Impact analysis gives no result for some patches, eg. the Details button does not appear for certain patches and its not showing the impacted files.
Checking the subrequest logs will show the following message:
'WARNING: Patch metadata LDT file not loaded for patch <NUMBER>'
The issue can be reproduced at will with the following steps:
Navigate to OAM --> Site Map --> Maintenance --> Patch Wizard --> Recommend/Analyze Patches --> Tasks
Errors may include:
Phase : COMPLETE, Status : WARNING, Msg :Program completed successfully but some rows have errors - check log for details, RetVal :True
ERROR: Failed to Upload .ldt for bug <NUMBER>
WARNING: Patch metadata LDT file not loaded for patch
Truncation of attribute VERSION value
