Enablement Service Pack 3 (12.1.0.3) cannot be applied to Knowledge Center (Doc ID 2187763.1)

Symptoms

When trying to apply the 12.1.0 Enablement Service Pack 3 (12.1.0.3) for Knowledge Center, after selecting the 12.1.0.2 installation directory, an error message is shown:

"ADO SQL object could not connect to the specified source.  Error: Login failed for user 'EMPTY'"

or

"ADO SQL object could not connect to the specified source. Error: null password given; login denied"

Followed by a message the the ESP must be applied to an installation of 12.1.0.2 or higher.  Installation cannot continue at this point.
