"Bill To Address" Is Blank On Service Request Form-Charges Tab (Doc ID 2193771.1)

Symptoms

On : 12.1.2 version, Service Request Form In Test and Production environments

ACTUAL BEHAVIOR
---------------
Bill to Address" is blank on Service request form Contact /Address Tab and also Charges tab

EXPECTED BEHAVIOR
-----------------------
Expect the bill to address should be not null

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Open Create Service Request form
2. Enter Account and Address details
