Intermittent Error: ORA-20002: [WF_INVAL_USER_ROLE] Assigning User Roles After 11i Patches

Since the application of a number of patches to the production system there have been considerable numbers of errors seen in user provisioning. During role assignment we receive errors not seen until the patches were applied.  The error is:

Oracle.apps.fnd.framework.OAException: java.sql.SQLException: ORA-20002: 3825: Error '-20002 - ORA-20002: [WF_INVAL_USER_ROLE]
ORA-20002: [WF_INVAL_USER_ROLE]' encountered during execution of Rule function 'WF_ROLE_HIERARCHY.Cascade_RF' for event '' with key 'FND_USR:74051|UMX:0|2457708:55243'.
