Web ADI Document Creation Page Failure: StringIndexOutOfBoundsException String Index Out Of Range

Symptoms
When selecting the "Create Document" function with the Desktop Integration or Oracle WEB ADI responsibility, a blank page appears with the following error message:
ERROR
The following error has occurred
String index out of range: -1
Log File Bookmark: 98620
WHAT TRIED
We tried the solution mentioned in the article "Create Document in R12 Web ADI Menu Generates HTTP 404 File Not Found Error (<Note 549501.1>)" and it only worked for a few hours.
