Problem Statement
  When trying to add an item from the catalog to the shopping cart, the following error occurs.

  oracle.apps.fnd.framework.OAException: oracle.jbo.DMLConstraintException: JBO-26048: Constraint "PO_REQUISITION_HEADERS_U2" violated during post operation:"Insert" using SQL Statement "INSERT INTO PO_REQUISITION_HEADERS_ALL(ORG_ID,AUTHORIZATION_STATUS,TYPE_LOOKUP_CODE,ATTRIBUTE_CATEGORY,REQUISITION_HEADER_ID,PREPARER_ID,LAST_UPDATE_DATE,LAST_UPDATED_BY,SEGMENT1,SUMMARY_FLAG,ENABLED_FLAG,LAST_UPDATE_LOGIN,CREATION_DATE,CREATED_BY,PCARD_ID,APPS_SOURCE_CODE,ATTRIBUTE1,CONTRACTOR_REQUISITION_FLAG,ACTIVE_SHOPPING_CART_FLAG,CONTRACTOR_STATUS,SUPPLIER_NOTIFIED_FLAG) VALUES (:1,:2,:3,:4,:5,:6,:7,:8,:9,:10,:11,:12,:13,:14,:15,:16,:17,:18,:19,:20,:21)".

Steps to Reproduce

1. Find an item from Stores.
2. Click "Add to Cart" to add it to the shopping cart.
3. Click "View Cart and Checkout", when click "Checkout" button, the above error occurs.
