Address Lines On Service Request Form Getting Duplicated (Doc ID 2225865.1)

On : 12.1.3 version, Service Request Form

ACTUAL BEHAVIOR
---------------
In the 'Service Request' form customer address is shown incorrectly in the Service header - some values are copied into another empty fields.
Address Line1 is copied into Address Line2 and Address Line3 is copied into Address Line4

EXPECTED BEHAVIOR
-----------------------
Custom Address entered in the one field should not get copied into another field

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to Service (R) -> Service Requests -> Create Service Request
2. Enter the instance number and tab out. Customer Details populated
3. Clicked on the Address details.
