Account Policies to Reset Password Are Not Applied To Few Users and Login History is Not Recorded (Doc ID 2226819.1)

Symptoms

For Few Users in OTM, the expiration date and last login date are exactly the same and are always updated to the last login date (Though expiration should be 90 days after reset date according to the Account policy). Login History in not being recorded. If an Admin User resets the password for these users , Users are not forced to change their password when they login.
