Symptoms
When attempting to run RGOPTM module: Program - Optimizer, the following error occurs.
ERROR
LEDGER_SEGMENT GL_CODE_COMBINATIONS_NSEGMENT Error
rgomid() encountered an error returning from index_not_created:
ERROR: ORA-00955: name is already used by an existing object

STEPS
The issue can be reproduced at will with the following steps:
1. Responsibility: General Ledger
2. Navigation: Reports -> Request -> Standard
3. Run Program - GL Optimizer
4. Notice the concurrent request ends in error
