"UnableTo Authenticate Session" Error in Self-Service (SSHR) Pages (Doc ID 2241248.1)

Symptoms

Problem Statement:

Using Employee Self Service or Manager Self Service, clicking on almost any link ends in error.

 

Exception Details.
oracle.apps.fnd.framework.OAException: Could not load application module 'oracle.apps.per.selfservice.hierarchy.server.HierarchyAM'.
at oracle.apps.fnd.framework.webui.OAJSPApplicationRegistry.registerApplicationModule(OAJSPApplicationRegistry.java:279)
at oracle.apps.fnd.framework.webui.OAJSPApplicationRegistry.registerApplicationModule(OAJSPApplicationRegistry.java:78)

oracle.apps.fnd.framework.OAException: Application: ICX, Message Name: ICX_SESSION_FAILED.
at oracle.apps.fnd.framework.server.OAExceptionUtils.processAOLJErrorStack(OAExceptionUtils.java:1006)
at oracle.apps.fnd.framework.server.OAApplicationModuleImpl.processAOLJErrorStack(OAApplicationModuleImpl.java:4604)

Steps to Reproduce:
The issue can be reproduced at will with the following steps:
Responsibility used: Manager Self Service
1. Navigation: Manager Self Service >> Absence Management (or any other link)
2. Observe Error
