WLS Admin Server Fails To Start With weblogic.security.SecurityInitializationException Error And BEA-090403 Authentication For User Weblogic Denied (Doc ID 2241778.1)

Symptoms

Customer is following <Document 1484024.1> to setup OAM/Webgate/Accessgate to integrate with EBS 12.1.x. In Step 4.4 Configure Response Headers -- when we try to access OAM Console, Launch Pad, navigate to Access Manager > Application Domains > Search for {Identifier for your WebGate}

There are no domains listed there.

We tried to enable diagnostic trace for OAM following <Document 1495179.1> but got no new details in the logs. When we  shut down the 3 managed server WLS_AMA1 WLS_MSM1 WLS_OAM1
and the WLS admin server, and tried to restart the Admin server. Admin Server now fails with the error:
