ORA-01403: no data found error when creating item instance in Install Base (Doc ID 2254137.1)

Symptoms

When attempting to create item instance in Install base for a party+party site combination users are getting the below error

ORA-01403: no data found in Package CSI_ITEM_INSTANCE_PVT Procedure CREATE_ITEM_INSTANCE_PVT

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Install Base Agent User
2. Select Create an Item instance
3. Create an item instance
Due to this issue, users cannot create an Item instance and put the item in customer's location
