After a upgrade to 12.2.5 and 12c when attempting to update user information via navigation System Administrator > Security > User > Define form (FNDSCAUS.fmb), the following error occurs:

'Unable to call fnd_ldap_wrapper.update_user due to the following reason: ORA-20001: PL/SQL - Init Failed (User_Name=UserName)'

The issue occurs after integrating applications 12.2.5 with OID via the following document:

The issue can be reproduced at will with the following steps:
1. Login to the system administrator responsibility.

2. Navigate to System Administrator > Security > User > Define form

3. Update a user record from the form.

The issue has the following business impact:
Due to this issue, users cannot update user information in forms.
