Note Entered By Approver Entered Via Email Is Not Recorded In Action History (Doc ID 2256395.1)

Symptoms

When a requisition is approved or rejected and a comment is added via email, the Note entered by the approver is not recorded in the action history Note column

 STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. User submits Requisition or PO for approval

2. Approver receives email and they approve or reject with Note entered in the email template response.  When entering the Note they see the '' single quote symbols and enter a Note after the '' Symbols and then enter a new double quote " symbol and submit the email    EG it looks like this ==>   Note ''Approval entered by Approver"  where the first '' are the single quotes and the last " is a double quote manually entered by the approver

3. Confirm the notification, the Note is missing from the Action History Note column for the Action just performed
