Web ADI LOV's Are Not Displaying in EBS R12.2.6

Symptoms
Fresh install of 12.2.6
When he creates a seeded Web ADI template, such as functional actual - single, and then tries to open any of the LOV's in the template, the icon's for the LOV's don't exist.
If they manually enters the correct data and uploads, the upload will work just fine.
For some users, the seeded template works ok but not a copy of the template.
For some users, both the seeded template and a copy the template does not work.
