20001: Non-ORACLE exception Error on Final Approver for Appraisal

Symptoms
On : 12.2.5 RUP8
When attempting to approve appraisal the following error occurs.
The changes were not applied because change_appr_status log=Success^&&apply_to_personal_profile log=&&update_train_component_status log=&&create_event log
=2 - Exceeds Expectations^13-APR-17^Success^&&error is= -20001: non-ORACLE exception

STEPS
The issue can be reproduced at will with the following steps:
1. Login manager self-service and submit the appraisal to final approver.
2. Login as final approver and approved the appraisal
3. After 5 minutes click on the notification again -> Error can be seen
