When trying to approve Purchase Order

Problem Description
---------------------------------------------------
The following error appears in PO:

Cannot find Combination CCID= XXX... The item field is then greyed out

ERROR
-------------------
APP-FND-00756: Cannot find Combination CCID=123 CODE=MSTK SET=456

STEPS
-------------------
1. Login to the E-Business Suite and choose the Purchasing Module.
2. Create new PO
3. Attempt to approve, and error is thrown

BUSINESS IMPACT
-----------------------
Due to this issue, users cannot approve PO
