ACA 1094-C Accepted With Errors Status with Error Codes 1095C-010 and 1094C-055

On : R12.2 HRMS Rup3, Affordable Care Act
Following Transmittal of Manifest and 1094 Request file to the IRS, received a status of "Accepted with Errors", however unsure what to make of the errors.
2 errors are noted in the IRS Acknowledgement file:
1. Error Code: "1095C-010-01"
Error Message: "Form 1095C 'OtherCompletePersonName' and 'SSN' in 'EmployeeInfoGrp' must match IRS database"
2. Error code: "1094C-055"
Error Message: "If Form 1094C checkbox 'AuthoritativeTransmittalInd' is checked and checkbox 'Section4980HReliefInd' is checked
and none of the 'ALESect4980HTrnstReliefCd' in 'JanALEMonthlyInfoGrp' through 'DecALEMonthlyInfoGrp' have a value,
then 'ALESect4980HTrnstReliefCd' in 'YearlyALEMemberDetail' must have a value"
Steps to Reproduce:
The issue can be reproduced with the following steps:
1. Submit Manifest and ACA 1094 Request Files to IRS
2. Receive acknowledgement email from the IRS with XML File that contains the above errors.
