PIM After Change Request, Workflow Is Not Getting Submitted to next Step (Doc ID 2274427.1)

Symptoms

ACTUAL BEHAVIOR
---------------
Workflow is not getting submitted to the first step in the workflow.
This is behavior for different workflows of different change requests that has different roles assigned at first step.

EXPECTED BEHAVIOR
-----------------------
Workflow is getting submitted to the first step in the workflow.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:

(1) Request for New Items

(2) Status:Approval

(3) Action of Step:2 is "Submitted", but Action of Step:1 is "(null)"
Notification did not submitted to Step 1.
This means no approval request at step 1.
