Symptoms
Randomly, when attempting to login, or after a timeout the following error occurs.

Error while invoking Main Menu. Application: FND, Message Name: FND_GENERIC_MESSAGE.
Error while invoking Main Menu. Application: FND. Message Name: FND_SESSION_MGR_INST_ERROR.
Changes
This particular issue occurred after DB upgrade, however other types of upgrades have been known to cause this issue too.
