1094-C Submission in "Accepted With Errors" With Error Codes 1094C-028 And 1095C-039-01

On 12.1 HRMS RUP9 version, ACA 1094-C IRS Submission
Following Transmittal of Manifest and 1094 Request file to the IRS, received a status of "Accepted with Errors", however unsure what to make of the errors.
In addition to the errors below - we are seeing multiple files being produced from the Benefits ACA Transmission XML Generation Process.
Two errors are noted in the IRS Acknowledgement file:
1. Error Code: "1094C-028"
Error Message: "If Form 1094C checkbox 'AuthoritativeTransmittalInd' is checked, then 'TotalForm1095CALEMemberCnt' must have a non-zero value"
2. Error Code: "1095C-039-01"
Error Message: "Form 1095C 'CoveredIndividualName' and 'SSN' in 'CoveredIndividualGrp' must match the IRS database"
Steps to Reproduce:
The issue can be reproduced with the following steps:
1. Submit Manifest and ACA 1094 Request Files to IRS
2. Receive acknowledgement email from the IRS with XML File that contains the above errors.
