On : 12.1.3 version,

Service Request created from MFS wireless have no bill to nor ship to address

Service Request created from MFS wireless should have bill to and ship to address

The issue can be reproduced at will with the following steps:
N) Field Service Wireless responsibility -> Action -> Create Service Request
1. Enter serial number, contact, summary, etc.
2. Save
3. Service Request in created.
4. Navigate to the Service Responsibility -> Find Service Request
5. Click on the Contacts / addresses tab -> All fields in the tab are empty
