Counter Capture Block On Service Contracts Form Does Not Open

On : 12.1.3 version, Service Authoring
Counter Capture block on Service Contracts form does not Open
when entering the readings for the Usage lines from Counter Capture while creating service contracts, an error is encountered
FRM-92101: There was a failure in Forms Server during startup. This could happen due to invalid configuration. Please look into the Web-server log file for details.
The issue can be reproduced at will with the following steps:
1.Contract Administration->Launch Pad
2.Search Contract : xxxxxx
3.Click Lines->Go to Counter->Focus on the Counter Line->Right Click->Select Counter Capture
