Appraisal Report EmpReport Having Missing Questionnaire Section.

While creating Appraisal Report EmpReport we are missing Questionnaire section.
The issue can be reproduced at will with the following steps:
Step-1 As Supervisor/Manager performing Appraisal for his staff
Step-2 Selected the Effective PMS Plan and choose ‘Manage appraisals’
Step-3 Choose the Staff and click ‘Appraise’
Step-4 Choose ‘Edit Appraisals’ and the Manager fill in the ‘Performance Rating’ and ‘Appraised Performance’ for Competencies and Objectives.
Step-5 Choose the Questionnaire i.e ‘Training Needs’ button and fill in the ‘Questionnaire’ details and choose ‘Apply’
Step-6 Choose ‘Save as Draft’ and ‘Continue’
Step-7 Choose ‘Apply’
Step-8 Choose ‘Provide Overall Rating’
Step-9 Then Manager choose the ‘Final Rating’ and choose ‘Share with employee’ Button
Step-10 Then choose the ‘Complete’ button
Step-11 You have choose to complete the appraisal >>Choose ‘Yes’
Step-12 Login as ‘Employee/Appraisee’ and notice a Notification
Step-13 Choose ‘Perfomance Management’ function through ‘xxx Employee Self Service’ responsibility
Step-14 Choose ‘Manage appraisal’ from the above screen
Step-15 Choose the ‘Print’ button
Step-16 Choose ‘Check All’
Step-17 Choose ‘Move All’ and choose ‘Include Questionnaire’ option and finally choose ‘Print’ button
Step-18 Choose ‘Save As’ from the toolbar as displayed
Step-19 Generated the PDF report and noticed that the ‘Questionnaire’ section is EMPTY
No Overall Score (11.95 + 69.85) displayed in the ‘Overall Rating’ section
‘Oracle’ logo to be replaced with ‘xxx’ logo
