Isupport Page Html Width/Alignment Issues after Upgrading to 12.2.5 from 12.1.3

ACTUAL BEHAVIOR
---------------
After 12.1.3 to 12.2.5 upgrade, the following html elements alignment issues:
1. In Page "Create Service Request" ( where the user selects the request type), the drop down for "Template" is in the middle of the page, where as in 12.1.3 it is left aligned
2. In page "Create Service Request: Define Problem" field "Create Service Request: Define Problem" size has been reduced by half
EXPECTED BEHAVIOR

HTML elements should align properly.
STEPS

The issue can be reproduced at will with the following steps:
Upgrade 12.1.3 to 12.2.5 / Apply patch 22956921.
