UPK: Errors When Publishing "Not all of the content could be published." (Doc ID 2318445.1)

On : version, Publishing Wizard

When attempting to publish content,
the following error occurs.

Not all of the content could be published. To see details about the content, click the "View activity log" link.

The activity log shows errors like
Stylesheet could not be loaded:
Root element is missing
An error occurred while loading document
The stream cannot be opened for reading:
Input could not be opened

The issue can be reproduced at will with the following steps:
1. Select module or section to publish
2. Click the publish button or choose file->publish
3. select "Publish entire outline" in wizard, click next

The issue has the following business impact:
Due to this issue, user cannot complete publishing.
