Open Form Error FUNCTION_STARTUP_ERROR After Upgrading to R12.2 (Doc ID 2319155.1)

Symptoms

 The problem started as soon as customer finished the upgrade to 12.2.6.  When user logging in System Administrator and opening the form Concurrent > Manager > Administer,  it gives the message:

FUNCTION_STARTUP_ERROR(FUNCTION=Administer Concurrent Managers)

and then directs the user to the System Administrator > Define Profile Options function.

All users  are impacted when opening any forms, it gives the message:

FUNCTION_STARTUP_ERROR(FUNCTION= XXXXXXXXXXXX)

The "FUNCTION=" part of the message will change depending upon what form you try to open.
