Symptoms

CSXSRISR Form(service request form) on Workbench tab under Notes section doesn't show note descriptions and note details after upgrade from 11.5.10 to 12.2.6


Steps to Reproduce
===============
1. Navigate > Service > Service Requests > Create Service Requests > disable custom code > create new SR > save
2. On Workbench tab under Notes section create new note also enter description save notice description field NULL


What Works
==========
Navigate to Tools1 > Notes description is displayed as expected
