When trying to open the Create Service Request form, several pop-up windows open with the errors below.
FRM-40105: Unable to resolve reference to item CONTROL.INCIDENT_URGENCY_DESC.
FRM-41045: Cannot find item: invalid ID
(N) Service Requests > Create Service Request
As soon as user tries to open the form, error appears.
Applied a Service cumulative patch.
