Symptoms

Running Autoconfig on DB or APPS Tier fails with error:

Context Value Management will now update the Context file
java.lang.Exception: Failed to add customizations
  at oracle.apps.ad.tools.configuration.FileSysDBCtxMerge.updTmplwithDBdown(FileSysDBCtxMerge.java:428)
  at oracle.apps.ad.tools.configuration.FileSysDBCtxMerge.updateFileSysTemplate(FileSysDBCtxMerge.java:286)
  at oracle.apps.ad.tools.configuration.FileSysDBCtxMerge.updateFileSysFiles(FileSysDBCtxMerge.java:211)
  at oracle.apps.ad.context.CtxValueMgt.mergeCustomInFiles(CtxValueMgt.java:1820)
  at oracle.apps.ad.context.CtxValueMgt.processCtxFile(CtxValueMgt.java:1635)
  at oracle.apps.ad.context.CtxValueMgt.main(CtxValueMgt.java:787)
Error occured before Context Value Management could be completed
ERROR: Context Value Management Failed.
