TeleService: Task Type LOV Showing Null (or) Does Not Default with % To List All Task Types (Doc ID 2340951.1)

Symptoms

ACTUAL BEHAVIOR
---------------
a) In create Service Request form, when clicked on Task Type LOV, it does not show the full list automatically. Have to manually enter %, then click on Find to get the full list.

b) Task Type LOV is showing null and not showing any data.

This issue is started happening after applying the CS RUP Patch 26522836:R12.CS.C - 12.2.4/12.2.5:BUG FIXES FOR CS:AUG '17.


EXPECTED BEHAVIOR

The task type LOV should get defaulted with % and populated full list values when immediately click on the LOV.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Apply the 12.2.5 RUP Patch 26522836:R12.CS.C.
2. Navigate to (R) Service -> Service Request -> Create Service Request.
3. Create the SR and click on Tasks tab
4. Click on Task Type Lov
